Transition
Royal School Manchester recognises the importance of creating bespoke, person-centred packages which support all of our children and young people to experience positive micro and and macro transitions.
Our vision is to ensure each young person’s strengths, aspirations and what is important to them is celebrated and used to inform their planning and preparation for adulthood.
Preparation is fundamental in understanding the needs and views of the young people at Royal School Manchester to support the development of their person-centred plan. In year 9, the Individual Education Plan will be supported annually by a person centred review process; including aspirations, barriers and desired outcomes relating to education, employment, independence, health, friendship and community. This review process is continued throughout key stage 4 and Post-16 whereby, once a future destination is identified, a bespoke transition plan is put in place alongside the young person.
Definitions of key terms:
Micro transitions: Small scale transitions for example between activities and locations; this will also include slightly larger changes such as moving from one school year to another.
Macro transitions: Large scale transitions or changes for example, from one educational/care setting to another, or the transition into adulthood.
Person Centred Planning: Person centred planning (PCP) is a set of approaches designed to assist someone to plan their life and supports. Contrasts with ‘system focused’. It puts the focus on the person - their hopes, dreams and aspirations at the centre of the planning.
